  • Registration form (Due 4 weeks prior to defense)
  • Dissertation Preparation and Deposit Instructions
  • Return Voting form after Defense to Office of the Registrar (Ann 12-80)
  • Exit Survey (Exit interview with Dr. Hanss)
  • Survey of Earned Doctorates form (print out completion page)
  • Student Checkout Form (clearance from all departments)
  • Complete Deposit of Dissertation – see above Instructions
